Yes his first name is John (which was revealed in the very last episode). He got his nickname, Mr. Big for...different reasons. I don't remember exactly , but I am pretty sure Carrie meets him in the very first episode, and shorty after she gives him his nickname.

Previous Yahoo Answers post says "His name is John. He didn't tell her what his name was in the last episode. In the end of the last episode he called her on her cell phone when she was back in New York and he was selling his house in Napa and his name came up on her cell phone."

she called him Mr. Big, because he was mysterious and rich. had a lot of power in life, traveled and drove around in a limo half the time.
