Gandalf goes to the Eastern Shores because he fulfilled his purpose in Middle- Earth, which was to see that the Ring of Power was destroyed. Frodo goes because his quest to destroy the Ring changed him so much, that he no longer feels he belongs among his fellows in the Shire. It changed him, and tired him so much, that he needed to go to a place of rest, and the Eastern Shores were a sort of Paradise where he could live the rest of his days in comfort. Same for Bilbo. As for Elrond, Galadriel, and her husband-- they are Elves, and, therefore immortal. Since the age of the Elves was over, they-- as the leaders of the Elves in Middle- Earth-- left to the Eastern Shores to leave Middle- Earth to be ruled by Men. What a great ending.

they're taking an Elven Ship back to Tol Erassea, as close as Noldor elves can get back to Valiinor. Basically they're headed to the paradise close to where the gods live. The elves are going back because they have grown weary of middle earth, Gandalf is going back because he's really a Maiar spirt and he can finally head home, and the hobbits are going because the age of the elves and their rings was passing in middle earth, and they got to go along as a reward for their deeds, and for healing from their burden of carrying that which they should never have had to- pretty unheard of for mortals.

They're tired out from saving Middle-Earth, and are being rewarded with eternal rest across the sea in Valinor. It's not easy being a hero, especially for these guys.
