A general rule of thumb is 6-8 weeks between sizes.

From 20-18 is actually a minor stretch, as is 18-16, and 16-14. You could get by with about 3-4 weeks for each of those. 14-12 isn't too bad, but 12-10 is where you start to notice the difference in how it feels, and how tender it is afterward, and for how long. 10-8 I found to be one of my more painful stretches. 8-6 wasn't bad, nor was 6-4. 4-2 was my first stretch where I used hollow earlets, and left my ears a little extra tender. 2-0 and 0-00 were about the same...a bit more intense than previously, and I gave each about 4 months in between (maybe it was more).
All of that was about a 3 year span. It was another year before I decided to go up to 3/8" and another year still before I went to 7/16" That one hurt.

The following years I went up and down for various reasons, but currently, after almost 11 years, they're at 5/8" and I'm happy with that (for now). They've been that size since July.

A big thing to remember is to go only one size at a time. It's least painful this way. It also keeps the healing/recovery time to a minimum. This isn't to say that it allows you to speed up the process...it just keeps it minimally traumatic to your ears and your own personal comfort. More than one size in one try can cause the skin to tear which makes for a potential infection site, and also prolongs the healing in that the skin has to first heal back together, and then get strong enough to stretch again. If you go gradually, it only has to stretch and get stronger again.
Also, similarly, you really should allow ample time between stretches because without enough time for the skin to toughen up, it's almost the same as skipping sizes, with the same potential hazards.
Finally, when you do each stretch, keep the new jewelry to something lightweight. The expanded hole is stressful enough to your ear, the added weight is additional aggravation you don't need until the tenderness goes away.
The good news is that when you do stretch, it stays tender (generally) for only a week, sometimes less.

my piercer tells me to wait 3 weeks, but you have to listen to your body. If you areent completely healed in that time, then go when its healed. I wait 6 weeks to be safe. i'm at 7/16 gauge now...
