'Shemale' is simply a term used in pornography to refer to pre-operative female transsexuals who have not yet had bottom surgery to remove their penises, but have some version of breasts, whether they are the those grown from estrogen, or due to implants (in porn particularly, they are most likely implants). They are usually on hormones to have a more feminine appearance.
Therefore, in order to become a 'shemale', you should start taking female hormones and possibly get breast implants.
