If you are going on a cruise from the continental US to Hawaii (or any cruise destination), you will soon be required to show a government issued passport (according to Carnival Cruise Lines http://www.carnival.com/CMS/FAQs/New_Passport_Requirements.aspx?wt.mc_id=Hcruiseclickspassport) even if you are an U.S. citizen.
However, if you are an U.S. citizen flying from the mainland to Hawaii (and I believe to Alaska also), you will only be required to show a picture ID such as a driver's license or state ID (with photo) for anyone ages 18 and over. Passports are also accepted. For non-US citizens, passports and sometimes visas are required. If you are flying to a foreign country directly from the continental US or using Hawaii as a stopover, then you will be required to have a passport.

I live on the Mexican border and starting January 1, 2008 you will need a passport to cross the border here. I don't know about Hawaii, I'll have to find out about that. Lots of people head this direction to vacation and in the process go over to Mexico to pick up their prescriptions for much, much less. This has been very good for our local economy. A passport is expensive and will put a stop to a lot of this. We are told it is to stop terrorists. I find that pretty stupid. If you look over the bridge, there is a stream of very wet people wading to the US. I doubt terrorists will come over a bridge. Since many people get their prescription drugs from Canada & Mexico, I wondered if the US drug companies are behind this move.

You will need a passport to travel to Mexico, Canada, or out of the Continental United States. I'm not sure whether a passport will be required to travel to U.S. territories or to Hawaii.

You will need one to come back after going out of the USA also, but I do not believe this applies to Hawaii - - not continental US, only other countries such as Mexico and Canada.

No. The Canada part might be true. I would not want to leave the US with my passport anyways.

Yes. And it does apply to Hawaii.
And picking nits here: you won't necessarily need one to travel out, but you'll need one to get back in.
