The W4 form.
If you have no tax liablity and will get a refund of all of your federal taxes you paid in, you can be exempt from federal tax.
Employers are not liable for their employees choices on the W-4; It's the employee's responsibililty.

The only form that's going to work for them to stop taking ANY federal taxes from your paycheck would be a letter of resignation because it's a federal law that your employer must abide by. You can change your number of dependants but cannot stop the employer from withholding taxes completely.

An employer that does not use independent contractors (under form1099) is not going to do that. They manage payroll to not put themselves in the way of being audited or hassled by the IRS. Having employees requires an employer to take out taxes.

In order to be an independent contractor, you must use all your own materials and equipment on the job. That includes everything from your computer to paper and pens. That's just one of the conditions of being an independent contractor.

I'm don't think there is a form to stop the fed tax but you can make changes to your exemptions on the w4 form to decrease the amount of taxes being deducted.
