My question was not answered the last time.

Usually, they don't strip you in ordinary private hospitals in India unless necessary. Is it necessary to strip my friend for putting a rod in his lower leg where he had a fracture?

Also, I haven't asked him, but since he has a plaster on, does his attendant strip him for defecation? Does he has to be wiped by the attendant too? And does someone need to change his underwear too while he is in plaster?

For most surgical procedures in the U.S. you will be placed in a surgical gown before being taken back to the operating room. Typically you will remain completely covered until after the induction of anesthesia. After you're asleep, you will be exposed in whatever manner is required for the surgery. Privacy is always respected, so if you don't need to be stripped naked to perform the surgery, you won't be. Sometimes it is required, though.

In terms of postop care for a tibial fracture, like it sounds as if your friend has, he should be able to get around just fine postoperatively. He'll likely be able to bear weight pretty soon after surgery. In the initial postop period he may need to use a bedpan, but this seems unlikely to me. He'll likely have very little problem getting up and moving around and I highly doubt his surgeons will have a problem with it. The sooner you get out of bed after surgery, the better unless there is some specific contraindication to doing so.

for a lower leg fracture, i doubt his cast will extend all the way up past his thigh, so he should be able to urinate and defecate by himself. as for changing his underwear, he can probably do it by himself assuming he's not like morbidly obese or has some other impairments.

as for the surgical repair, he should be able to keep his underwear on- but like you mentioned, every hospital has their own regulations pertaining to sterility in the operating room. he may be forced to strip because of hospital regulations, but he shouldn't have to just because of the surgical site.
