I suppose you're talking about Franklin D. Roosevelt, not Teddy?
FDR was elected for four terms, actually, but he passed away very early in his fourth term.

President Roosevelt was elected to office in 1932 and became the 32nd President in 1933. He was elected to 4 terms but only served 3 full terms. He died during his 4th term for unknown reasons. He was a polio victim and had to use a wheel chair which is unknown to most people. He was one of the best Presidents this nation ever had! He died in April of 1945 before the end of the Second World War.

Elected to four terms in office, he served from 1933 to 1945, and is the only U.S. president to have served more than two terms. During his fourth term On March 30, 1945, Roosevelt went to Warm Springs to rest before his anticipated appearance at the founding conference of the United Nations. On the morning of April 12, Roosevelt said, "I have a terrific headache." He was to never speak again.

Yes, FDR actually served 4 terms from 1933-1945. He did die in office, just 2 wks before Germany surrendered in WWII. It was after that, that the 2 terms policy went into effect.
