In Buffy the Vampire Slayer, Tara died from a gunshot.
Warren went on a rampage and attempted to kill Buffy in her backyard by shooting her. One of the shots went wild and killed Tara in the house.
(Unfortunately for Warren, Willow was present during all this and ended up holding Tara as she died. I say unfortunately, since Warren was [in effect] the cause of the Event that made Willow become "dark, creepy Willow" [per Xander])

Tara died by a stray bullet that was meant for Buffy. Warren (part of the trio of Buffy's "arch-nemesises") stormed into Buffy's backyard shooting at her. One of the bullets shot into Willow and Taras room and hit Tara in the chest. It was so sad because Tara dies in Willows arms right after they just got back together. Willow begs for Tara's life back but since she died of a natural death it can't be done, causing Willow to turn to dark magic, turning her into "scary black eyed, dark haired, veiny Willow" She later seeks her revenge by killing Warren and coming after Jonathon and Andrew.

tara died wen dat guy (u noe, black haired, wizard,tryin to take over world, had 2 friends who were blond) came to kill buffy. he fired 2 shots. 1 hit buffy & d other went through d window and shot tara.

she got shot by accident by one of the three geek/super-villians that plagued that series.
