Gravity. There is (theoretically) a super-massive black hole at the centre of the Galaxy, and the stars, gas clouds and dust of the Milky Way are all orbiting within its gravitational field.
It's like the Solar System, where the planets orbit in the gravitational field of the Sun, but scaled up.
There is some dispute over the structure of the Galaxy, however, as the laws of gravity that we are currently using predict that there should be a lot more mass within the Galaxy than we can see. This is how the theory of Dark Matter came about, to account for the 96% or so of matter that we predict but cannot see.

Like all galaxies, the Milky Way is held together by gravity. This gravity also holds the stars, gas, and dust in orbit around the center of the galaxy. Just as the planets orbit around the sun, the sun orbits around the center of the Milky Way.

A Gravity well formed by the supermassive black hole at the galactic core. The discovery of stars that have reached escape velocity form the galaxy may mean that there are 2 supermassive black holes at the core.

.........well.............the milky way doesn't need 2 b holded together...............because it's all vaccum there out.........it's just a big group of stars (including us) standing nearby each other........bcoz thir formation took place at the same time leading to such an arrangement

I think the theory is that it is some sort of anti-matter. This stuff supposedly explains why the universe is accelerating outward (which goes against the big bang which would tell us that the universe should be slowing down).
