That is a pretty general question -- you don't even tell us what field you are in.
I treated doing my dissertation the way I would treat a job. I went into the office five days a week. I sat at my desk and worked during the day. In the late afternoon or early evening, I went home. I did very little work at home other than download data.
I set deadlines the way I would have with a job. My dissertation was in finance -- and we usually write three papers. For each paper, I had a hard deadline. The first one had to be done in the middle of my third year in order to send it to a conference so that I could present it during the summer. I also needed it for my Oral Exam. I finished it just in time, and got to present at a conference in Amsterdam.
The second paper had to be done by October so I could use it as my job market paper. I finished it with about three weeks to spare.
The third paper needed to be done about two weeks before graduation. It was.
If you are almost done -- come up with a timeline for finishing it. Make sure that you talk with everyone on your committee to make sure that they are on board. You don't want any surprises at your defence.

Don't freak out when you do your dissertation defense. For your first defense, choose the worst faculty thugs in the department, and get them out of the way. You might even want a thug as your advisor -- that way you're guaranteed a thumbs up vote for that idiot for your defense, and all the wimps will be easy to defend for.
