It means the amniotic fluid sac or membranes has ruptured. Sometimes there is a gush of fluid, sometime there is only a tear and subsequent leakage.
Sometimes the sac ruptures at the onset of labor, sometimes not for several hours after labor commences. In either event, once the water breaks, its time to go to the hospital as injury or infection are more likely without the additional protection for the baby.
