Try The Gap, J Crew, American Eagle, Banana Republic -- all stores that are in most decent-sized malls. Also there's Old Navy stores that are separate locations. And then there's always the LL Bean catalog.

Pretty much any store. Scour the clearance rack of good stores to get nice ones; Gap, Banana Republic, Holster, American Eagle, Sears, K-Mart, Walmart, Macy's...
