The focus of this assignment is probably on two things . . . on the "autobiography" part . . . and on the "essay" part. Have you learned about how to write an essay? It has an introduction, then there are several paragraphs in the "body" of the essay, and then there is a conclusion. And each paragraph deals with only ONE topic.
Now for the "autobiography" part. It's about YOU. So maybe there will be three paragraphs in the body of your essay; the first one about your early life, the second about your current life, and the third about your future.
This is just an idea of how to approach this assignment. You might want to consider going to the teacher after class or after school if you have further questions. Hopefully, your teacher has set aside hours to be available personally to students who have questions.

This kind of writing is going to be about YOU
So only you can really write it. Suggestion:
Where and when you were born, and what kind of family.
Then go into your early years . . . where you went to your first school, anything that was specially interesting (maybe your parents won the lottery, moved house, or bought horses? ?)and how you went into middle school?
Then your middle and high school years - did you have a hobby (Soccer, juggling or something), did your family have major changes (divorce, death of a relative), did you yourself have major changes (broke a leg, won a pageant or spelling bee or whatever)???
Then today what is going on - where are you spiritually, physically and mentally.?
Only you can get this information onto paper and the three sections can be fascinating and very revealing. Winning a competition can influence your later life and be a "defining" moment, as can a geographical move. Adding small details j("our chihuahua dog won the 'smallest dog in town' three years in a row at the July 4th town picnic. then we decided not to enter her any more - not fair. Stuff like that)

How about: I was born on May 26, 1998. Or whatever your birthday is. Then write about your childhood and go on from there. It shouldn't be all that hard since an autobiographical essay is all about YOU
