Intro-- write a lead in sentence, something exciting that captures your attention, then a sentence that summarizes the entire essay, then a sentence summarizing each paragraph you write to support your general thesis topic
Supporting paragraph--state what you will seek to prove (what you want to say) then provide at least two supports for that.
Supporting paragraph--state what you seek to prove then at least 2 supports
(optional) 3rd supporting paragraph
Conclusion-- summarize what you've already said, then introduce a tidbit of additional information

Its easy to write a five paragraph formatted essay...
1. Intro- introduce the topic and slightly touch on supporting details
2. Supporting detail 1 - details supporting your topic
3. Supporting detail 2 - " "
4. Supporting detail 3 - " "
5. Conclusion - tie up all the loose ends and end your essay
