We procrastinate when we are supposed to be doing something we don't really want to do. So resolve you will not promise to do something you don't really want to do, and then no one will be disappointed by your putting things off.

Stop using the word procrastination and the bad feeling that is associated to that. Choose what you want to do in life and do it with passion. Give up the actions that other people think you should do, but you think them unnecessary. Make a list of the things that if you won't do, they will later affect the things that you like (e.g. going to the dentist). Have a plan to do them and enjoy the time after they are finished.
Use your time for yourself!

There's really no way to fix it other than sheer force of will. Most procrastinators know that they are going to have a hard time later, but put it off anyway. It's not like it's a disease; it's what you want to do and the only way to change that is to convince yourself that you want to do it now instead of later.

make a schedule for yourself. make yourself plans so that you have to get your work done before you leave. stick to it, and force yourself to accomplish things even if you don't want to. You really have to be motivated to fix it though.
