I've been a chef for over 20 years. Within that time I have never worked with anyone that had thought of or commited such disgusting acts to an order like I've been reading here or seen on that stupid movie! So, I'm curious if anybody has actually seen this activity and what did you do?
I think for myself, I would probably get violent and beat the living sh** out of you (if I don't stab you first)

Yeap, I had a friend who was a cook on a ship and he and the rest of the galley staff didn't like the way one of the other crew was treating them, so they always gave them the really small serving. This didn't get it through so he yelled at them one time for not having anything out for midnighters and told them to make him something like a steak cos he knew there were leftovers. Well this guy had already thrown them out but the idiot was yelling and really wanted it so my mate took it out of the rubbish and cooked it for him. ('Marintated' and all)
Serves the idiot right.
PS. This cook is normally one of the most placid guys so for him to do this was massive, I couldn't stop laughing when I heard! (also because I knew the guy he did it to but so funny)

Nope. The closest I have ever seen is someone dropping a raw steak on the floor, then washing it off before cooking it, and the occasional handling food without washing your hands first. (And to be honest, that has happened to us all.) But I have never seen (and I also have 20+ years experience) anyone purposely tamper with a customers food. You are exactly right, it would probably cause me to go ballistic.
I have seen, however, people tamper with other employees food, never anything nasty and disgusting, just the occasional introduction of some hot sauce or the like into a coworkers drink, or dish. All in fun, of course.

Kinda. Working in a deli and when a customer would ask for somehing like 10 chicken breasts all in separate bags this person would make sure there was a hole in every single bag before she wrapped them up!
