It could mean several things not just shouting at you. They might really want you to understand what they are saying, do it for attention, to emphasize what they are saying etc...

It means they are trying to emphasise the word or the sentence so the reader would get the point clearer.
It might mean they're angry or just want to get attention quickly. It works.
