I once worked in transmission shop. One of the new fellas (my nephew actually) attempted to pull a filler tube (thin metal tube about 1 inch in diameter) out of a transmission sitting on the floor, but it was kind of stuck. So he grabs it with both hands and pulls as hard as he can, suddenly it lets go, and he ends up sticking it right into the middle of his forehead. He had to go to the hospital to have it stitched up. A few days later he removes the bandage and there's a perfect circle (like a plug) stitched into his forehead. Everyone at work razzed him about his unicorn operation.
He still bears the scar today 32 years later.

I work at a tutoring center. Recently a father came in to pick up his son. His kid was busy reading a paper while walking out. The father opens the door for the kid who promptly walks into the other door!

when I was in Jr High this kid ate a lunch bag full of gummi worms. He ended up puking them all over the hallway. He had been trying to make it to the bathroom so when we went to change classes we saw these little piles of gummi-puke all the way down the hall.

funniest thing that has ever happened to me is that i got into a fight when i was in flight school which wasn't that long ago and got pushed down a flight of bleaches. (i was safe OK)
