I would guess that in some small, very rich country, where everyone would get a check from the government whether they worked or not - like Brunei or Qatar. If I'm not incorrect, everyone in Brunei gets an annual check the equivalent of US$25,000 from the Sultan. Pretty cool.

A communist government.
You have the same opportunities as your neighbor. One of those opportunities is choice. You can choose to work and feed your family, or choose not to and your family will starve. If you have needs(food, clothing, housing) you choose to work until those needs are fulfilled. If you no longer need these things, you can choose not to work at all.
If you choose to be lazy, you will never fulfill your needs, and no one is required to give you food or shelter, since you yourself decide not to work. No Public Aid in this government, no free rides on the backs of hard workers. Elderly and orphans are housed and fed by the government, if their family and friends won't or can't provide for them, but able ones are required to do some kind of work collectively and the product is pooled to feed everyone in the institution. No second helpings, and if there is overflow of money, the unused portion is fed to other institutions in need. Usually, these people work in harder jobs than people who are not dependant upon charity, since there are usually more who cannot work for themselves because of age or infirmary. Once able-bodied and of age, these people are set out and must work for themselves, wherever they choose to live or build a life, according to their needs.
Everyone is paid the same. And you work depending on your needs. There is always jobs for everyone who needs to work. You can work a land, but the land belongs equally to everybody, and everyone involved in living on the land must work together in order for there to be enough food and shelter for all the workers and each worker works according to their family needs. Lazy, hostile people can and do get driven out.
You can choose to get an education, or not. You can choose to privately be religious, but if you go around bothering people with your religion, you could be arrested, which would put your family in jeopardy, since you will not be working. In a case such as this (or committing a crime) it is up to the balance of family members to work and feed themselves, or, assuming your wife is not physically capable (say, broken leg and blind) and your children are too young to work (say, under 5 years old)you may be issued work while incarcerated, so that you can provide for yourself and family while imprisoned, or your children can be sent to orphanages if the other parent cannot provide for them.

In another form of communism all the above is the same except you are required to work in a community, and everyone in that community splits the product of their work equally. This form is less productive, because a worker who has 1 family member gets the same as a worker with 10, and so of course has more for his work, which is similar to many capitalist governments.
All in all, communism is the most equal-opportunity and equal income form of government.

there aren't any that I'm aware of...

but there are several in Europe that are much closer to equal... heavy taxes on the wealthy and large social programs have almost eliminated poverty in some Scandinavian countries... but the wealthy still have quite a bit more than everyone else...
