Water/moisture will travel through concrete. If the ground under or on the edges is wet, the floor may sweat. If this is the source as opposed to surface condensation, you will begin to get salt deposits on the concrete. It'll be a whitish dusty film. If it's a concern you can clean the surface and seal it.

Generically it is because the floor is colder than the air above it, and that air contains a good bit of moisture.

You want to look for the source of moisture. It could come from an improperly vented gas appliance, such as a furnace or water heater.

What part of the country are you in? Maybe a local contractor has a sollution--my concrete breezeway sweats and I always thought it was because I was in the South.
