the condition is called diprosopus. It happens when twins start to form and the egg doesn't completely separate.

The same things as for Siamese (conjoined) twins, except at a lower scale. Some bits of the embryo got a mangled chemical signal that they were both supposed to be the face, and developed accordingly.
