I'm sure you have tried the conventional cleaning methods and the problem has persisted........the problem is urine is water and it has probably seeped into the flooring i.e. under the tile and into the sub flooring. remove the linoleum or tile and allow the area to dry, then apply bleach mixed 1/4 cup of bleach to 1/2 gal. of water ...again let dry, sprinkle with a light coat of baking soda and let this sit for a day. vacuum thoroughly then lay tile and be sure to seal the grout with a liberal coat of clear acrylic. also be sure to caulk the edging around the baseboards to prevent urine from seeping back in behind the edging....last paint the walls around the toilette with a washable glossy latex after washing it with bleach solution this is the guaranteed way to remove the odor and prevent it from returning

Take old rags and place around the toilet where grandpa misses. Either wash them frequently or throw them away. To help with the aroma, you can place a shallow dish of vinegar near by to help absorb the odor. The bathroom might smell like vinegar but it is better than smelling like urine

Well, it seems that the problem is not removing the smell, but the cause for it's repeated return. If grandpa sits down, it will not be an issue anymore. The biggest question then, will be:
"How do you ask/convince him to sit down?" Good Luck!

Also, those industrial bowl hanging deodorizer blocks. They have them at Dollar Tree stores.
