If by every case you mean every case the Supreme Court agrees to hear then most of the time yes, but there are occasions when they don't. Each individual judge can decided to remove themselves from cases that they have a personal interest in, have ruled on in the past in a different capacity or if for any reason they are incapacitated, i.e. too sick to participate. No other justice can order another justice not to rule on a particular case. If the court ends up tied the lower court ruling will be upheld.

No, they choose the cases they will hear and rule on based on relevancy and immediacy.

if the cse is hear, yes. but the supream cort does not take every case that is submitted. they can be remanted back to the original court.
