To answer your question...
No.
The earphone and microphone jacks are soldered on the mainboard. Replacement requires a new mainboard.

Replacement of the fan/heat-sink assembly requires complete dis-assembly of the laptop and should only be attempted by a qualified technician.

Your XPS may still be under warranty, I recommend you begin by searching for any recall notices or warranty information that would apply to the issues you are having.

You can replace the fan fine, but the headphone jack is built onto the motherboard and cannot be replaced without pretty good knowledge of electronics and a new jack. You will probably need a whole new motherboard.

Can you replace the fan assembly WITH the earphone and microphone jacks?

You could probably replace them individually, but I probably wouldn't replace the fan with something completely unrelated to it...

i wouldn't, without the fan your computer will probably overheat. You could try vacuming out your fan or buying a new one. For headphones and microphones, you could probably just buy a cheap video card and put it into a PCI slot.
