Only happened once on Rainbow Six Vegas. I had only been playing for maybe 30 minutes. I have my 360 in a well ventilated spot.
Any thoughts or suggestions?

First off, are you having any problems with other games? Also, how long have you had the 360? I started getting those errors more frequently on my system, and eventually the drive quit working and I had to send it to Microsoft for repairs. If you have had your system for less than 1 year, Microsoft just recently updated their warranty program to 1 year. I would suggest calling them about it. If it's only happening on the one game, then the disc could be faulty. Try getting another game...

I had it happen 2 times with my Gears of War disc, but never after that (it was about 3 weeks ago). Maybe there was just some dust on your disc? I just wiped it off after and never had a problem again. If it starts happening a lot more it you might want to use that extended warranty ;).
