The "5 Rights" of medication administration are basic principle taught to clinical personnel, including nurses and pharmacists. There are additional rights that have been introduced throughout the years, but the basic principles are a standard part of medication administration, saftey, and best practice.

The basic 5 rights are: Right Medication, Right Dose, Right Patient, Right Route, Right Time. These rights were established to provide a standard for medication safety and best practice.

JCAHO is an accreditation organization. They coined the term "sentinel event", which describes an adverse drug event resulting in significant harm to a patient. However, I can't recall JCAHO being responsible for coining the "5 rights."

Most of my co-workers state that the "5 rights" are part of best practice learned as part of their basic clinical curriculum in school. In recent years, there have been references to a few additional "rights." This article attributes the additional rights to specific sources.
