Well, even the contraceptive makers admit that not all the contraceptives are 100% perfect and their is always a chance of failure. This can cause pregnancy inspite of protection. But that is 1% chance.

No birth control is 100% safe unfortunately. Even the pill or a condom that didn't break are 99.something% safe

because using condoms are not 100% protection. Condoms do have minute pores. others sperms may spill.

There are a lot of possiblities like the condom broke, she didn't take the pill soon enough, she doesn't react to the pill... who knows?
