There are many factors why people don't have as many children as before:
1.There are great contraceptives in the market.
2.Women's evolution in the society. Now women work and they are not only at home being moms. Women usually like to be settled as a professional to start having kids.
3. Money. Now, couples usually wait longer to have babies, because they want to have enough money to maintain the children.
4.The cost of life is higher now, so couples don't want to have a lot of children in order to being able to provide them (and themselves) all that they need.

Health care and far fewer problems with child mortality play a role in this decision, too. Many generations ago, parents had to bear 7 children just to ensure ONE child making it all the way to youth/adulthood, such was the terrible toll that child mortality took on families. But nowadays, parents don't have to worry so much whether their kids will make it, so they don't have to give birth to as many.
Also, generations ago, many more families than there are today were occupied with agrarian (farming) forms of livelihood. More kids meant more helping hands. So they had an incentive to have lots of kids. Whereas nowadays, how many kids do parents need in their high-tech, computerized lifestyles, living in modern settings? Certainly not seven!!
"And just remember, the best thing about kids is making them." (Rodney Dangerfield, in "Back to School" 1986)

today most people wait longer to have kids, they wait til they are settled so that gives them less time since the fertility rate goes down after the age of 35 for both men and women
also, way back contraceptives like birth control, etc. were not available

Having many kids used to be a necessity back in the day. More help to run the house, help out on the farm, etc.

Because they have been scared by their parents and society that sex is evil and that if they have kids their life will be ruined. This is a lifestyle choise not a biological phenomenon
