In the olden days, single people at a party wore a valentine-style heart on their sleeves at some parties to designate that they were single and interested in meeting people of the opposite sex. They were "wearing their heart on their sleeve".
Today, it means that a person is showing their romantic vulnerability. If you say, "I am wearing my heart on my sleeve", you're saying that you're letting everybody know you are looking for someone.

a person who is sensitive and usually puts their feelings and emotions our for everyone to see. they can be vulnerable, the type to be careful to say "i love you too"..the type who move to fast...they dont protect their feelings and emotions on the inside, so they wear them on the outside (on their sleeve) which can cause emotional pain, sensitivity, and misery for the person.
Wearing your heart on your sleeve is a common phrase.

I think that's relative to I wear my heart on my sleeve, which is basically someone who is very outgoing and shows who they are outfront- they don't hold anything about themselves to be secrets.
