I didnt file this custody change document with the courts yet....would it still be valid if i filed it now?

Theres also a stamp by the notary that says "my commision expires oct 14 2006".... is this the time its valid to?

also:.... The document does not have any other date as to when it was prepared or effect date, etc.

A document, once notarized, remains valid. The notary's term can expire, but the stamp on the document does not. For filing, check with the appropriate court.

There should be a signature & date on the document.

I would guess that would depend on what your state laws say.
