As a mother of a 16-month old, and being 3 weeks away from giving birth to my second... I can honestly say that the best gift to help out would be... help!
Frozen meals (such as casseroles, homemade chicken pot pie, lasagna) with explicit directions how to reheat are a great gift. Another good gift is someone to come over and clean the bathroom, vacuum the floors, or any other mundane household task that you never have time (or energy) for. It's even free for you to give - just head over and say you're going to clean the bathroom or mop the floor... don't ask, she'll likely say no out of embarrassment or something. If you don't live locally, a great gift is a gift certificate to a maid service such as Molly Maid.
Another great gift is offering to take the kids out to the park or on a short excursion (of an hour or so) with the explicit instructions that Mom takes a nap. Even taking the older child out to the park for a longer time so that Mom can have a nap with the baby would work great.
Now.... do you know anyone who will come over and clean my bathroom? ;-)

I just had my 2nd child, she's 2 months... I'm not exactly sure what you are asking but I can tell you what really has been useful around here. A bouncy and a swing. Also a snuggli or a sling! That way she can hold the baby(no hands) and be able to do things around the house. Diapers are always a good thing, but my little one grew so fast I just had to exchange 3 packs of sz 1 diapers because she outgrew them, people still give them to me! I'm not complaining I think it's great. And I totally don't mind exchanging them. A bottle rack and nipple brush are good to have too. I've also heard great things about BUMBOs(litle seats the baby sits in)!
and if you want to get her something cool.. I loved the little ceramic hand print kit I got at my baby shower!

Go to her house, ooooo and ahhhh over the baby for about 5 minutes then get up and put a load of laundry in to wash, wash the dishes, spiff up the bathroom and cook some dinner and run the vacuum over the carpets.
Trust me, this is the best gift you can give.
If you really must wrap something a nice basket from Bath and Body works and a gift certificate for some place that can deliver dinner is nice.

A good present for a new mommy that already has one is a day off. Go online and make your own gift certificate to her saying you will watch 1 or both of her kids for her so she can get some rest, go out, etc....I know that's what I am hoping for, I'm pregnant with my 2nd child right now!

There are so many things that a new mother needs. Since she is a second time mom its hard to say what to get her since she might already have alot of stuff that she needs. You can't go wrong with a gift card. It would be nice to get one to wal-mart or somewhere to buy diapers and stuff like that. Or you can make your own little gift basket with necessities like diapers, wipes, balmex, baby lotion, shampoo, etc. Or, you can get stuff for her. Like bubble bath, a book. Something to relax her and giver her a night off. Maybe a day at the spa!

Offer to watch the older child or both children so she can have some alone time, just to take a nap. Do house work for her (laundry, cleaning, cooking), bring her some food that will last a few days like a large casserole, do some grocery shopping for her. You can always just ask her what she needs.

Well, I think meals are the biggest help personally, or gift certificates to take out/delivery restaurants. But if you want a baby-ish thing, I appreciated getting some new basics. Everyone assumes that since you already have a baby you are set for sheets, changing table covers, onsies, etc, but truthfully those things are often quite grungy by the time number 2 comes along. I loved getting some new ones.

In my opinion it would be a day for herself. With two children, mom might forget about herself and spend all of her time focused on the children and father. Time for "mom" doesn't even have to be that long, but maybe a nice bubble bath w/a magazine and some scented candles would be nice. But really, any kind of help offered would be a great gift for a mom.

Diapers are always a useful gift- if you are looking to get the mom something for herself consider a gift certificate to a spa and the offer to watch the baby! She may not be able to use it right away but a couple of months later the pampering will be greatly appreicated.
