No one does, but the president can appoint someone.
Its not like the presidency where there is a list of who comes next.

My US government is a little rusty, but I'm pretty sure the President appoints a new vice president. If the President dies, and the VP replaces him, then he would appoint the new VP, so it's not very likely that the speaker of the house will ever be the President (at least by those means).

Under the 26th Amendment, it's the speaker of the house who is next in line to the president, BUT the president gets to appoint a new vice president with the advice and consent of the senate. In 1973, with Spiro Agnew facing time in Maryland on a fraud conviction, President Nixon appointed Gerald Ford, then the House minority leader, to be the vice president. A year later, in August 1974, Nixon resigned and Ford became the president, not the then Speaker of the House, Tip O'Neill of Massachusetts.

The President simply appoints someone else. The new vice president has to be approved by a majoority in both the Senate and House of Representatives.

The line of succession, several people have mentioned, is in case of some major disaster where most of the government officilals are dead.

If the two the President and vp of the are no longer able to serve, the Speaker of the domicile takes over as President. Then it somewhat is the President professional Tempore of the Senate, accompanied through the Secretary of State.
