The #4 bearing and rod went to pot on my car. During repair the mechanic found out that my car is not supposed to have an EGR Valve, yet a hole was bored in the head. Is this a factory defect, or is it possible to have the casting plug fall out while driving?

I'm going to assume that you have a 2.4. No it's not a defect: 99 was the transition year for the engine and GM used up the last of the EGR heads. The fact that the manifolds don't have concessions for the EGR means that it doesn't matter if the head does.

No trouble there intake covers the hole and its a normal thing.
