The phrase came about in the Middle Ages from the practice of young men and women drawing names out of a bowl to see who their valentines would be. They would wear the name drawn on their sleeve for a while (I've heard any time from a week to a year).

I pretty sure it makes a reference to Hawthornes, scarlet letter. the lady in the novel committed adultery and had to wear the letter A on her sleeve. A is a symbol for adultery obviously, but also for what she did and what she felt. Sometimes "you do what you feel in your heart" meaning what you're feeling.

It means you may be a little too apparent in your emotions and not able to hold back that you really like someone or be able to act like your feelings are not hurt when they were.
Usually taken to mean that you're not in complete control of you emotions.
