These are places/websites with boots for wide calfs:
1. J. Crew (www.jcrew.com right now has a sale on some extended calf boots; not sure how wide the calf is, though, on them).
2. www.silhouettes.com

5. Sometimes, Ann Taylor and Ann Taylor Loft have wide calf boots

Torrid and J. Crew have boots for thick calves. Other "plus sized" stores would work too. Additionally, look for lace-up boots.
