how is the speaker of the House picked; why did we know it would be Nancy Pelosi imm. after Nov. election?

The Speaker is picked by the House itself through a vote. It is common practice to vote in their minority leader as Speaker once they obtain the majority. No one challenged Pelosi, so she got the job.
