No, it is not real. Understand that behind the cameras are literally dozens of people - directors, cameramen, technicians of one kind or another. It just doesn't happen. In fact, when it supposedly does, it makes news. No, the sex was not real for Monster's Ball, either, though I'm sure there are many people who wish it was. However, back in 1990 Mickey Rourke and Carre Otis were suspected by the crew of having real sex during one of their scenes. From Wikipedia's article on Rourke - " It was during production of Wild Orchid that Rourke began a relationship with his co-star, Carre Otis whom he later married. Rumors that a love scene they filmed for the movie was not simulated have persisted to this day, although both actors deny it."
So, for better or worse, the answer is simulated.

Probably fake. The only mainstream movie I heard of that claimed to have real sex was "Last Tango in Paris". I didn't see it, but I seem to recall that the reviews said "Save your ticket money and go buy a regular porn flick. It probably has a better story and plot". When the producers have to resort to this kind of gimmick to get people into the theater, it's because a movie can't do it on acting, plot, writing, etc.

For the most part it is simulated...But there are exceptions. The awful movie Brown Bunny has very real oral sex scenes.
This was bout 99.9% responsible for the downward spiral of Chloe Sevegeney (sp?) who at one time was an up and coming actress.
The same is not true for a number of your mainstream European (mainly French) movies which often have "genuine" sex scenes .

Some recent movies have employed real sex on screen, however, they are typically indie films. The most notorious of them is "Shortbus" which had all sorts of sex acts, all done for real by the actors. "The Brown Bunny" had a scene in which Chloe Sevigny performs a certain act on Vincent Gallo. A movie called "9 Stories" also had the actors engaging in real sex. The director was Michael Winterbottom, who also made "A Mighty Heart."

for the most part, it's simulated. they cover the more sensitive portions with plaster or some other material that can't be seen while they're "doing it" in the movie

If Hollywood sex were real, there would not be a porn industry. It's fake and they usually were a modesty shield over the parts you can't see.
