Are you are asking how does she know if she's pregnant? Her body is sending her mind signals of the changes taking place in her body. Even before the first physical symptoms appear she will instinctively know or "feel" that she is pregnant.
If you are asking how does she confirm those feelings? Get a drugstore pregnancy test. Then take her to a dr. to confirm or disprove the home pregnancy test results. These tests are not 100% accurate.

Start with a pregnancy test kit, most of them are sold in pharmacies, and they detect the hormone secreted by the future placenta (HGC) or human gonadotropic hormone in the urine of the future mother to be.
They are useful (the home test kits) from one week after the expected period (the misssed period). names or brands of the kits are "Predictor" or "Preg-test" or Pregnosticon, available over the counter in most pharmacies.
However, if still in doubt, get an analysis of your wifes urine in a proper lab. asking for the Antibody test for pregnancy (the test looking for HG alfa subunit antibody test) and that is quite reliable
however that may cost you between 35 to 46 US dollars, depending on the location of the lab.
Lets start with the simple home kit test, and we can start from that.
PS: be careful to follow all the isntructions written on the label of the kit, otherwise there is a small risk of obtaining a "negative" false result, please read them carefully with Your wife, knwing how the test (very simple) is done before doing it,

Go to a clinic and let her test her urine for pregnancy or go to a drug store and get a pregnancy testing kit and test yourselves.

The first thing to look out for is a missed period. Has she missed her period, or experienced tiredness or breast tenderness? Then she has to take a pregnancy test to find out for sure.
