When you see that on a legal document it usually refers to the fact that all terms (full disclosure) are written within the document. No additional terms and conditions exist at the time of the contract (no verbal agreements).
It can also mean that you are avowing that your answers/statements are true and that you are fully informing the bank of all your income/assets & debts and other statements you made.

Am not a lawyer or accountant, but disclosure on financial accounts means you must declare any and all assets, investments, monies you have , you owe.
