It is about "The Patient" who dies of Cancer and his greatest memory is of his father taking him to a parade therefore when death comes for him, it comes as a black parade. Somewhere along his journey through death, he comes across "Mother War" who basically brings up alot of issues he had throughout his life that he has to work through to carry on to the afterlife.

Well, if you have listened to the album, it is built around someone called "the patient" and his young, unexpected death. And, the song, "The Black Parade" Gerard sings as if he is the patient, of course, and says: "When I was, A young boy, My father took me into the city, To see a marching band, He said, 'Son when you grow up, will you be the savior of the broken, the beaten, and the damned?' " So, my take on it was, gerard is saying the patient went as a boy to see a parade, so as the patient is dying. he is thinking of that parade, he is also thinking about death, being a black parade. Therefore, the title, The Black Parade. So, I guess that is just maybe what I think, or maybe that's what other people think too, whatever it is, it is definetly a good song ! Hey, do you like My Chemical Romance, or just that song ? I like both ! Good Luck on finding your answer !

The Concept of the song 'Welcome to the Black Parade' is that the patient who is dying of cancer is about to die. He believes that death comes for you in the form of your fondest memory. His memory is going to a Parade with his father when he was younger (Idea comes from Gerard went to the Macy's Day Parade with his father when he was younger) So Death comes for this man (the Patient) in the form of a "Black Parade". While journeying through 'the black parade' he is given a medal which is thought to be a symbolization of death.

Well, My Chemical Romance believes that death comes to you in the form of a memory, and Patient, who is the main character of the song and the whole CD, is dying of cancer at a tragically young age, and the memory of going to see a parade when he was a young boy is coming back to him as he is in the hospital dying.

It's really strange I looked it up. Gerard (guy in the band) appeared on Fuse, and says thwe song is about a patient dying in the hospital. His fondest memory was when his father took him to a parade when he was a young.
Gerard believes that when you die, death comes to you in any form you want it to. So as this man is dying, remembering his father and the parade, death comes for him oddly in his fondest memory with a twist: a black parade

Gerard is refering to the Black Parade as death. He is saying that Death will come for you in a way that you want. In the "patient's" case in the video his fondest and most vivid memory was that of his father taking him to Parades when he was little. Therefore Death came to him as a Parade......the Black Parade.

Actually, they haven't said, so no one really knows what it's about.
There is a made-up story to it (The Black Parade is yet another concept album), but the actual meaning behind it is unknown.
The original title was "The Five Of Us Are Dying", so it must have something to do with MCR's life experience.
