It's interesting that it says "for better or worse" because that is exactly the opposite of what the vow should be. It is "for better and for worse." When things get tough, that's when the marriage needs to be at it's best. I agree that it should say "for better and for worse" because there are always those worse moments and that is when those promises come into the picture. In a marriage you agree to work together to get through those hard times.

Neither. Only for better, because that's how good relationships go. The vows might more sensibly include "through good times and testing times, as long as love lasts". It's foolish to jump overboard when the boat first springs a leak, but we only have one life and it's even more foolish to drown when bailing fails!

If the final modifier is "as long as we both shall live," then there's no difference between "and" and "or" in this useage.
"And," however, would imply a requirement that things be both better AND worse.
"Or" means I'll take it either way.
