This is too many people to play games. It wouldn't be fun for anyone, as they'd be standing around forever waiting for a turn.
Play music, have food and let people mingle.
Actually, I think this is way too many people, no matter what.

Here's an idea. Have a huge diaper cake made for the centerpiece. We're talking five or six tier. You could have a fishbowl where people could "Guess How Many Diapers in the Diaper Cake!!".
You could also do a pool on the baby's birthdate and time. With this many guests....I would divide the days around the due date into quarters. Everyone gets a dib on a six hour window!!
With this many people....the key is to NOT have games where individual responses are required. Stick to games where people guess things. All the people at this shower won't get a chance to talk and mingle. But lots will....so stick to a "social" theme and just have a few games because every shower should!!

Have you thought about diapercakes for centerpieces at each of the tables? For the seating chart you could have each table assigned a themed diaper cake (the teddy bear table, the froggy table, the giraffe table, the puppy dog table, the ducky table...etc). Each guest would then have their name written on a litle wooden figure matching the theme of that table's diaper cake and that figure would mark their place at the table....MUCH MORE FUN THAN TRADITIONAL TABLE ASSIGNMENT CARDS!!!! The gift table could have the big five or six tier diaper cake..... Just an idea!! (And think of all the diapers for the mommy to be.....split the cakes with size 1-3 and she may just be set on diapers for months!!!)

You can play guess the cottonball or cotton swabs. Put a counted amount of either in a large plastic baby bottle (party city) and have the guests guess the number in the bottle.
You can also play guess the candy bar in the diaper. This is really funny, especially with a lot of people. Set out 5 baby diapers, melt a candy bar in each one, and have everyone guess what is in the diaper. I used non-popular brands and we had about 50 guests, no one guessed all 5 right. We gave the winner a gift card.
Another great idea that the mom loved was when everyone walked in the door I had them fill out their name and address on the envelope of a thank you card, put it in a basket, and leave them there for the mom to send her thank you cards in so they were all already pre-filled! Then we did a drawing and picked a winner from the basket.
Also you can get a bunch of pink or blue clothes pins, everyone gets one when they walk in the door. If you hear someone say "baby" you can take their clothes pin. Whoever ends up with the most clothes pins at the end of the shower wins a prize.

The only thing i could think of is BABY BINGO! Make little playing cards and fill them with baby toys, daipers, lotion and soaps, powder, stroller, baby seat - different stuff by putting there pic's on them and the name below and draw the things in front like a bingo thingy- that'd be really cute. Lots of prep- but really cute!
Also if they're at like round tables- a friend had this one where they gathered some things in a basket and put it on the table and let us see it for say a minute or two and then gathered them up or covered it and then you write down how many names of the products you can remember- and whoever got the most things correct, won! well there's a couple - can't think of a whole lot that many people can do and not get bored. You might wanna think about having several- instead of one big one?! just an idea.
Otherwise remember the little prizes for the game winners. Might wanna think about little things the guests can have tied in to the baby as well. Depending on how money goes i guess.
I know a family member made little ribbons like for breat cancer awareness things with the little loop in the ribbon and made there's pink or a girl , but you could do blue for a boy- but she found little mini baby bottle things at a craft shop and put that in the center of it and just glued it together in a pin to wear. They were kind of cute. Nice momento for family members as well.

Fold white napkins in the shape of a diaper and glue on little blue/pink plastic safety pins. Inside 1 of the "diapers" put in a square of Hershey's chocolate, let it soften a little then squish it around. Under chair place a "diaper" and tell guests leave them where they are until told what to do with them. When your ready have the guest "check" their diapers. The guest with the "messy" diaper at our shower won a large bag of Tootsie Rolls.
The pacifier game is a good one too, so is the duckpond game, and "Guess What's in the Diaper". Games like those are great with large groups because you can be passing the diapers around while the mommy-to-be is opening gifts and you can announce a "winner" during gift opening time when guest are beginning to get bored.
Another good one: pass around various baby items and ask guest to guess the cost of them, the one closest to the actual total wins.
We did all these for our youth director's wife and gave away the floral center pieces and candles from each table as prizes, with winners picking the one they wanted.

You could have guests write down guesses at the beginning about how many of common gifts (diapers, bibs, pacifiers, etc) the new mom will get during the baby shower. That's a game that won't take them long to do, but will keep them around until the end.

Oh honey! Dont play games. let me tell you, I had my baby shower last weekend, with around 120 people and we DID play games and it was a disaster! It took so long to play and everyone was getting bored and not paying any attention anyways. Then when the game was over, we ended up with like 15 winners, because of the huge amount of people I had there. Then when it was time for me to open my gifts, I had SO many gifts that it took me 3 hours to open them and I was going as fast as I could. People were getting bored and most of them ended up leaving because my shower was taking so long.
Games are fun, but I would advice you not to play any with a shower so big. Good luck and congrats!

This was fun.... take some empty babyfood jars and put different things inside. ie., sugar, flour, powder, cornstarch, powder soap. Give each jar a # or a letter. Each guest must name each item inside each jar.
Take tiny safety pins and tie little bow's on them, pass them out to each guest when they arrive. Later, when you have most everyone there. Let them know that they are not allow to cross their legs or feet. Also they cannot use the word "baby" or they will loose there pin.
Dont forget prizes for the games....gift cards are cool.... buy several from different stores...display them on a table so everyone can see.... before you hand out prizes put them into a bag or basket.

A mason jar and 10 clothespins Let the person stand over the mason jar and see how many clothespins they get into the jar and whomever gets the most then they win a prize. Get a roll of toliet paper and everyone tears off how much ever they think the pregnant woman is round. The pregnant woman measures herself with the toliet paper then counts the sheets then whomever gets it right wins. Another thing is at the beginning of the shower the pregnant woman has something on like a necklace or ring then they take it off then the woman askes the people at the shower what is missing from her outfit.

The pacifier game! Get tiny plastic pacifiers (easy to find at a craft store) and tie them on string to make neclaces. Giev everyone a neclace when they come in and tell them the rules at the door.
Rules:
If anyone says "baby", "pregnant", or whatever you are naming the baby, or you can choose other key words, they lose their neclace to whoever hears them say it.
The person with the most neclaces at the end of the shower leaves with a prize!

Guessing the size of around the Mother's tummy.
Using a sheet of paper and crayon everyone has it on there heads and draw a baby the baby comes out the best wins.
