It depends on what State you are in. But in most, you have to petition your Parish/County court for a name change. It's really pretty simple. In most cases you can get the form from your local Clerk of Court's office, file it in proper person, meaning representation on your own without an attorney, pay a court fee usually without a court appearance, if all the documents are in order, get a signed Judgment.

Also, you may want to add into the petition that the Department of Vital Records be Ordered to re-issue a birth certificate with your new name.

If you are in the US you go to the courthouse for the county you live in pay the fee and file the papers. Once that is done you fill out all the forms for IRS, Social Security etc, just as you would had you married.

Petition the court for a name change. Call your local court house and ask them what you need to do. After you get your name legally changed you need to take the legal papers to the social security office to obtain a new social security card with your new name on it.

here in Australia if you are over 18 yrs old you can change your last name through Deed Poll (if under 18 you will need the permission of both parents if still living)....check with the Births,Deaths & Marriages dept in your area for more info...you will need a copy of your birth certificate

If you do it legally, then you have to go through the court. However there is no law that says that you can not be known by anther name. Legally you could be Herman Detwieller, but you could tell everyone that your name is George Lipshitz. Later if necessary the legal terminology is, "also known as".

If you want to change your name in NSW, Australia, you have to do it with the Registry of Births Deaths & Marriages department.
