1. Begin to mix breast milk and formula. Begin with 90% breastmilk, 10% formula. After a few days, when he has accepted that mix well, go to 80/20, then 70/30 until you get completely off the breast milk.
2. 4, 6 to 8 ounce bottles. Morning, noon, afternoon(like 4pm), and bedtime. When you begin to wean, take the afternoon bottle first, then the lunch, then whichever of the morning or night he/she cares less about, then the final one.
3. 7 or 8 months, but that depends on the baby. Some crawl soon, some late, some don't crawl at all, they go straight to walking. It's important to let him/her roll around and build strength. Don't feel like you need to hold him/her all the time.
4. Words should start soon thereafter...8 or 9 months. The babble of dadda, etc. At about 14 or 15 months he/she will be adding nearly a word a day to the vocab!
Most importantly, realize that every baby is different and the timing of things is different. I would recommend the book "What to Expect the First Year." It tells you month by month what to expect as far as development.

I have a six month old son. With regards to formula, I would try mixing it with breast milk, say 3/4 milk and 1/4 formula for a few days then gradually increase it until its straight formula. It could also be a simple case of nipple confusion. If thats it then you may have to buy a few different bottles until you find the one that your baby likes best.
In regards to eating, because hes still on formula, as he should be, he won't eat much. Introduce small amounts, mixed with formula, not water. If he seems to want more after he eats, give him more until he decides he's full. If you find that he spits up after, make a note of it, and decrease the amount as needed.
My son isn't crawling yet but my daughter didn't start crawling until a few days after her first birthday. All babies are different, so he will start when he is ready too. Don't be in a rush. Encourage him by placing his bottle, soother or toy just out of reach and see what happens.
His first word, like crawling will come when he's ready. My son still hasn't said anything. My daughters first word, however, came at 7 months old....she said dada.
I wish you and your son all the best. Remember, enjoy this stage of his infancy, it doesn't last long!

Try giving your baby some breast to start then when not so hungry try the formula? Has the baby drunk anything from the bottle? if not it may be the bottle they do not like.
At 6 months the current guidelines are to introduce solids so some cereal in the morning( 4-6spoons) after the bottle to allow the baby to intake enough formula inthe day.
Then more cereal say at 4pm with a fruit or veg. keep on feeding the milk on demand even if on a bottle. (or at least every 3 hours)
They start crawling at differnt times depending on the baby. One of mine did at 9months for two weeks and then was walking!
My 8 month old started crawling at 7 months and seems to progress on another major milestone within two weeks of achieving one, however my girlfriend has a baby the same age who does not move! not even a roll but was able to sit at 4 months! so it happens at different times.
My 8 month old said her first word at about 7 months I think

I've had three babies and nursed them all. I suggest mixing a little formula in with breast milk and keep adding a little more till he accepts it.
Your pediatrician probibly preferes starting food at 4 or 6 months. If you're just starting out, a tablespoon of rice cereal mixed with formula and/or breast milk once or twice a day is enough.
Your baby will start crawling when he is ready. To get him moving just a little faster you could do regular baby exercises with his arms and legs. Spending time on his belly will help too.
Don't be surprised if you don't hear a first word till 11 months.

Start mixing the milk 1/4 formula to 3/4 breastmilk, then slowly add more and more formula until that's all he gets (over a couple of weeks). I can't remember exactly how much mine ate at that age (my youngest is three months). You really should buy a copy of the book What to Expect the First Year. It'll answer all of your questions. Good luck.
Here's the book:

1. You can try mixing formula with breast milk. At 1st at half spoon to your breast milk. If he is taking it then slowly slowly increase your potion.
2. At 6 months, you can try to give him some rice cereal or porridge that cook till very very soft. At 1st give a few spoons to see whether he can take it or not. Everyday try till he get use to it. When he can take soild already then you can increase the potion to half bowl and so on.
3. That depends. Some baby are fast while so are slow. so just let your baby take their time. When they are ready they will start to crawl.
4. That depend also. But you can strat to talk to him like when eating say mum mum. When he always hear you say that he will learn to say together with you.

YOu should get the book "What to expect the first year". It is the "bible" of the first year of parenting and can answer many of your questions.
From personal experience, My six month olds all ate cereal and fruit for breakfast; for lunch a half jar of veggies and a half jar of fruit for lunch, and more cereal and a half jar of veggies at dinner, with half bottles to drink.
YOu have a while to go before you think about crawling. Not till usually 8 months at the earliest, though of course there are a few prodigies out there.
Only Mama and Dada and maybe one other word int he first year is my experience. 6 months is too early to expect even a meaningless "da da".
