I know that anything I will tell you will be easier said than done. But I can tell you what works for me. I study for about one week before a test, and on the day of, I generally don't try to learn anything else. It only causes more stress. (By the way, you study, but how are your study habits? Do you study effectively and retain the info well?)
When I get to the test, I take a couple of breaths, and remember it's too late to worry, it's "go time." If you run into questions you don't know, try not to let them get to you...think of all the answers you're sure of...and I think, "well, the worst-case scenario is I fail."
If you've failed a test before due to this anxiety, you see the world doesn't end from one bad test. So, that's a reminder that it's okay to miss a few questions, as long as you don't freak out from it...and miss all of them.

Well my mom had that exact problem when she was in school. I use to be the same way, I would get so nervous when I took a test that everything I studied went right out the window. I started noticing though that the less I thought about the test and just did the best that I could the more I got better grades.
If that doesnt help you could try something fun like making up songs to remember definitions and such. Most of my teachers let everyone listen to music during class when they werent talking or something so that helped ease the nerves alot. If you cant do that then you could just try wiring the headphones through your jacket if music helps calm you...
Just go in with a good attitude knowing you studdied and did everything you could do to be prepared. Its important to stay confident and think positive thoughts.
Some people just seem to get bye in life without any effort at all but with the kind of dedication you put into your studies I'm sure you'll come up on top above them all! hope that was helpfull... feel free to email me or anything if you have other questoins.

Perhaps the problem isn't the amount of study, but the method of study. Maybe you should try studying in some new ways. If you study alone, maybe you should try a study group. If you cram, maybe you should try periodic studying. As far as actually taking the test, I find it is always best to feel comfortable. Wear comfortable clothes and sit in a spot where you feel at ease. I always find a seat with some degree of privacy if possible. Keep in mind how much time you have and have an idea of how much time you'll take on each question. One last piece of advice, and it may sound stupid, but make sure you answer the right question on the answer sheet. If you skip a question, you could screw up all the rest of your answers. Good luck.

When you study, spray a certain perfume or cologne that you enjoy around you and your study space. When you go in to take your test, spray the same scent. This isnt really a calming suggestion, but I've heard it helps with information retention. You might also want to try and write a mock test, and take that in the safety of your own room before you face the real thing. After you take the test, make sure to reward yourself, with the satisfaction that you got through the test. If you do well give yourself a bigger reward. You are on your way to test success!

Yes I can't pass a test for nothing, I don't like test, especially those job tests that have nothing to do with the job in the first place. Drink some water before taking the test or chew some gum.

Eat chocolate or sweets before your test. This is not an uncommon problem and you can get help from your school counselor. They could set up oral testing for you and see if this helps. Don't let them tell you it is impossible,it isn't , if they do tell you no, get a prescription from your doctor. Good luck.
