It sounds like you basically have your answer already.
If there are, for example, 23 births per 1000 of population, then maybe you're supposed to convert that to .023 births per person (per capita), but there really isn't any calculation required.
Same for death rate.

per capita usually means per person or individual.
Of course, you must have a given data for the number of births and number of deaths. For example, if the number of births is 2,000 and the number of population is 200,000 then, to calculate the number of births per 1000 people as you indicated, then convert first the population to the corresponing multiple of 1,000 which is 200. Then, divide 2,000 births by 200 and you get 10 births per 1000 persons. Correspondingly, you can use the same data or a different data in calculating the number of deaths per 1000 persons.
