Is "significant other" good? I thought of "domestic partner" but that seems to be something you have to go ahead and make permanent.

What's more permanent than living in the same household together? How do you address each other outside of the house? In a couple of states, it's called common=law married. So, get your act together until those answers you offered are added to the forms.

If you are not married then put single. If the form asks the relationship of the person in question then put friend, or girl/boy friend.
