I believe it is like the Queen for a Day program. On the day that you receive the "Key to the City", you get a few perks such as a lunch in your Honor with the City fathers and mothers/big wigs, a chauffeured limousine ride, a plaque for your wall that has a "mini" version of the Big Key they gave you and took photos of.
I also believe that when you are in the City in the future, you are a welcome guest at any point in time and that the City will host you again.

The key is too big to unlock or lock anything. There could be some perks such as a meal with the mayor or a special place in a parade.

It doesn't unlock anything - it's merely a symbol showing the recipient that the city is 'open' to him or her, and usually there are quite a few perks that come along with it, such as free hotel, restaurants and sightseeing.
