The answer to your question is NO, you would simply list the last or current title you've got with the company.
For example, let's say that I worked at XYZ Corporation from 1999 to 2005. I started in 1999 as a Customer Service Rep, was promoted to Senior Customer Service Rep in 2001, and in 2003 became Customer Service Manager.
On my resume, I would simply put that I worked at XYZ Corp from 1999 to 2005 as a Customer Service Manager because that was the last job I had with them. But in the description of my job responsibilities, I could if I wanted, state the fact that I was promoted twice. That shows that I was recognized by management as being a good employee and have risen up the ladder.
But as far as putting your job title on there, you always list your most recent one!

That depends. If at least some of these positions were vertical movements (i.e. promotions) that is a good thing to put on the resume b/c it suggets increasing responsibility. On the other hand if there are too many horizontal movements (i.e. more than one each year and more than 5/6 overall)- it may suggest that you are incapable of staying on a job for long and would get itchy the moment you get hired without excelling in the current position.

If the positions have different job descriptions and if it gives details of your improvement within the company, then go for it. But if the positions simply different titles for similar tasks, then just list one title and include all the responsibilities.

Definitely, each position you held shows you have varied skills. As an ex-vp, I always looked for the most experiened person for the job or promotion. Showed me they wanted to excell above the others by gaining the most knowledge possible out of their job, position or assignments.
